❖ Form must be completed and submitted in person

❖ All documents must be attached together

❖ Registration & Resource fee will need to be submitted with the registration forms

Required documents to be submitted with the registration form:

✔ 1 Passport Sized Photo

✔ Copy of Proof of birth (Birth Certificate or Passport)

✔ Copy of Immunization and Vaccine Records

✔ Copy of Health Card

✔ Copy of the most recent report card

Please note:

❖ Late or incomplete applications would not be accepted

❖ The registration fees are non-refundable

❖ Sibling discounts only apply for Grades 1 to 8
